Award Amount: $5,000

 

Purpose: This scholarship supports academically driven students who show a dedication to furthering their education and positively impacting their communities, carrying forward Dr. Hayes’ vision of investing in the future.

Eligibility Criteria:

​

1. Academic Excellence: Applicants must be high school seniors or currently enrolled college students with a minimum GPA of 3.5. 

​

2. Citizenship: Applicants must be citizens of the USA. 

​

3. Community Service Involvement: Demonstrated commitment to community service, volunteerism, or mentorship, especially in areas that reflect the foundation’s values, such as supporting veterans, families, or community development. 

​

4. Educational Aspiration: Applicants should be motivated to pursue advanced education, with clear goals for how their studies will enable them to make a positive difference in their communities. ​

Personal Statement (500 words): Describe how education has influenced your life and future goals, the challenges you’ve overcome, and how you plan to contribute to your community through your chosen field of study. Share how you relate to Dr. C Medley Hayes’ commitment to “planting seeds for trees whose shade we may never feel.”

3. Letter of Recommendation: Provide a recommendation letter from a teacher, mentor, or community leader who can attest to your academic performance, character, and dedication to service.

4. Transcript: Submit a current academic transcript that confirms a GPA of 3.5 or higher.

 

Selection Process: 

 

The selection committee, comprised of CMH Foundation members and volunteers, will review applications, focusing on the applicant’s academic goals, community service involvement, and alignment with Dr. Hayes’ values of education and community commitment.
